Here's some places that have long-ago B&R information:

http://www.phyrexia.com/bandr.shtml back to 2000

http://www.crystalkeep.com/magic/rules/tournament.php back to June 1999

But that's all I can see... I can't find anything further back than 1999 (the official site's searches only go back to 2004). Hopefully that helps a little.

It is really a shame if there isn't complete info on this -- its like losing a piece of our history.  I've often wondered what was legal at a given point in time when I've seen old decklists.

It would be awesome if the DCI published an official record of the "history" of tournament magic, including the launch date of each format and the historical B&R list of each.  The DCI/WOTC must have a record of this.

If they won't do it, we could attempt to reconstruct this for ourselves, but memories are imperfect and not all of us (including me) have been here since the beginning.

As for erratas, does the Gatherer's historical information on each card go back to its [tournament] debut and reflect all changes going forward?

this is why wikipedia is so fantastic, it auto-records every change made. similiar database for magic should exist, even if not editable by public.

Fastbond was the most recent addition to the restricted list in October 1996.

In the January 1998 issue of The Duelist (#21, pg.34) is was reported that effective with the October 1997 B&R announcement: Candelabra, Copy Artifact, Feldon's Cane, Mishra's Workshop and Zuran Orb were taken off the Restricted List. I couldn't find any reference to any other changes in between those issues.

Later Additions to the Restricted List included:
January 1997 - Black Vise
January 1998 - Strip Mine
January 1999 - Stroke of Genius, Windfall and Tolarian Academy
March 1999 - Time Spiral and Memory Jar

All leading up to the big addition to the B&R list in October 1999 detailed on Crystal Keep

So Mystical tutor, vampiric tutor and enlightened would be legal? (Aswell as necropotence, but that one really didn't surprise me - They didn't take action until Trix rolled over every format it was legal in)

Correct.

Mystical, Vampiric and Enlightened Tutor weren't restricted until October 1999. Necropotence October 2000.
