If I have an Ophidian in play and my opponent plays Pithing Needle naming Ophidian thinking that it is an activated ability (like it is written on the card 0: Draw a card etc.) what would happen?

I know it is a perfectly legal play to name it (even though it won't do anything), but the question is, is a player supposed to be aware of all erratas on all cards? If he asks if it is activated, I know I have to answer truthfully, but let's say he doesn't ask since the card is cleary worded as an activated ability.

If he passes the turn only to learn on my turn that is Needle is worthless and that the gamestate cannot be reversed, it is one of those "Too bad, you should have known that errata" situation?

Yes.  If he is not interested in getting the Oracle Text from a judge and names Ophidian, then you can't play activated abilities of Ophidian.  You can still draw cards and your opponent can look upset all he wants, but there is nothing illegal about what he did.

Just so I get this correct.  Ophidian's trigger happens during the declare blockers step of combat.  It is then that the owner of Ophidian must decide whether to draw a card or not.  Do I have this right?

Indeed.  It is a triggered ability now when he 'attacks and is not blocked', which would happen in the block step.
