help me find a book

Kind of an out-of-left-field topic, but you guys might be able to help.

I'm looking for a specific children's book. I don't know the name or author, but it was pretty memorable.

It consisted of about 20 unconnected illustrations, each with a short caption underneath. They were generally spooky in tone, or mysterious, or both.

The one I recall the best (and probably the scariest) had a man holding a broom defensively, staring down at his carpet, underneath which was a lump. The caption read something like, "Mr. Jones held his breath and listened. He was sure he had seen it move."

Does anyone remember this book? Does anyone know who the author was, or the title? I remember seeing it in a store a couple years ago and reading the jacket - it's the only book the author has ever written, and IIRC he disappeared shortly after publishing it...? (Assuming the jacket actually claimed this, it may have been sales propaganda.)

I want to say the author's name was David something but I'm not at all sure of that.
