Say my opponent has Phage, the Untouchable in play.  I case Phantasmal Image, and choose Phage.  Do I lose the game?  I suggest not, because I did cast the Image from my hand.

Phage:

When Phage the Untouchable enters the battlefield, if you didn't cast it from your hand, you lose the game.
Whenever Phage deals combat damage to a creature, destroy that creature. It can't be regenerated.
Whenever Phage deals combat damage to a player, that player loses the game.

Phantasmal Image:

You may have Phantasmal Image enter the battlefield as a copy of any creature on the battlefield, except it's an Illusion in addition to its other types and it gains "When this creature becomes the target of a spell or ability, sacrifice it."

You don't lose the game, because, as you already pointed out, you cast you're Image from the hand.
