In following suit with OMGMSPAINT, here's my setup:

This picture isn't very to-scale, the library is on the edge of the playing area, and the RFG pile is usually right next to my deckbox, or so it just happens that way and I notice it.
And, I usually hold my hand in the left hand, or set it on the table. I really don't like it when people react to my draws behind me in a tournament and it's a semi-serious match. I Always do everything with my right hand; draw cards, attack, fondle little kids, etc. It's just old habit I guess

EDIT: Alright, image ain't showing up, but the library is on the Far left of the playing field horizontal, Top-loader facing away, Yard vertically underneath, in order and only showing card name, as to conserve space. RFG is the same as Library, but to the left of it. Cards are ALWAYS tapped in 90° clockwise fashion, and I love sorting my land into neat lil' piles. Attackers are put ahead of other creatures to signify when in the Attack phase. Artifacts and Enchantments are always directly to the right of the library, then follows creatures. I don't actively seek this setup, it's just what's natural to me. I also get really irked about people half-tapping mana. Tap that shit like it's hot, the whole way.
