If I target my opponent's Sanctuary with my Heretic, does he take 5 damage or does the Sanctuary see itself being destroyed and replace the damage on the way out?

You remove 5 cards. The destruction and the damage is a simultaneous effect and occur when the Sanctuary is still in play.

I kinda figured it would be something to that effect since Yawgmoth's Agenda "sees" itself being Disenchanted, et cetera.

5 cards are not removed.  The destruction and the damage are not simultaneous.  The actions are carried out in order as printed.  First the artifact is destroyed then 5 damage is dealt.  Since the artifact is no longer in play when the damage is dealt, there is nothing to replace the damage.
