These arent necessarily play mistakes.
My three worst are as follows.
#1 Consulting for a card not even in my deck. #2 Building a combo deck with no MD kill and no wish. #3 Playing City of Solitude in Enchantress after I had the game locked with Story Circle. My opponent promptly killed me.
Im sure you can top these. Lets have 'em.

My most stupid mistakes ever in a game... 1.) Over confident 2.) Not paying attention 3.) Not protecting the F2 square And I made all of these mistakes in the same game And after 13 years my Dad has never played me again (He knows he should have never beaten me), but he still will not let me forget that he wooped my ass in Chess in three freak'n move when I was fifteen! It sure is funny how parents can teach you lessons that stick with you forever, since that day I am never over confident, I always protect the F2 square, and most importantly, I never played my Dad in any game just he case he would be able to pull out a win.

I don't think I'll ever forget the time I was playing EBA against Mykeatog, who was using sligh. I had a morphling and an exalted angel out with around 14 life and he had 9 life, with force of will and a blue card in hand. He bolted the angel, and I forced it. He then price of progressed when I had 9 nonbasics out for the win.

Activating Memory Jar during my opponent's end of turn step. Yeah, let's just not go there.
Oh, and leaving the mountains out of my w/r draft deck was pretty bad too. Edit: No, I wasn't running 17 plains--I actually had a 32 card deck. It wasn't pretty.\n\n

Ha, ha, ha. These are good.
I had a disenchant in hand (and in plain view) while watching my opponent use a soldevi digger (artifact) to create an infinite loop... I didn't stop him; I guess I was amazed that that card could do anything usefull...

hmmm probably entering a five proxy tourney with long and forgetting the tendrils at home , I went off every time and lost. Also: I played a brainstorm and an ancestral recal while my opponent had a Monkey out ( Monkeys) and a Chains of mephistopheles. meh. also I did the same thing next turn except with even worse- a memory jar.(I hate chains of mephistopheles )

mmm.... 1) Forgot to attack with an sligh-kinda deck for 1 turn. (lost) 2) Builded a very cool deck that workt as if it was a multiplayer deck. 3) Forgot to draw a card for 3 turns
Some mistake i hope i will never make is this one (some people made them )

One time I Bolted a Brass Man, then Incinerated it in response. My thinking was:
1. I had just read about someone responding to his own kill spell with something else to RFG the Nether Spirit. 2. Hey, I'll do that too! 3. Wait, there's no point in removing Brass Man from the game. 4. Wait, Incinerate doesn't remove it anyway. 5. Fuck.

I once had an attacking Anger coming at me and the opponent had just cast a fresh Goblin Welder. Forgetting about Angers ability, and I was low on life (7), for some reason figured I would just block Anger to play it safe because I was able to win next turn. Anyways, this made the Welder active and I proceeded to die shortly after.....Damn Triskellion took out my Morphling and he won on his next turn!

I was sitting pretty in my first tournament with my Keeper deck, in a match versus a bad Reanimator deck. I had 5 Goblin Soldiers and was closing in on the kill, with 4 life to hold me for 2 turns. He cast a Doomed Necromancer, which I immediately Swords to Plowshared. He used its tap and sacrifice ability in response to bring out an Angry Iridescent Angel, and killed me. I was so excited about being there that it never dawned on me that he couldn't do that. He went on to the finals. Talk about a stupid mistake.

1)end of turn vampiric tutor, then in your end of turn sac fetchland....
2)I make 10 mana upheval atog play mountain discard al my hand (with anger...) and remove all graveyard, so attacck cause anger gives atog haste... but i removed it....
3)with survival of the fittest in play wait for turns for a creature with squee in my graveyard...
4)made a white weenie deck with red for 4 bolt and 4 shaman and 2 anarchy in sideboard...............

Worst mistake ever: I was playing against Zherbus in one of his concord tourneys for a mox emerald.
I had Academy Rector out, and therapy in the graveyard. I flash the therapy and start searching.
But i had Necropotence and Yawgmoths Bargin in my hand.

Forcing an overrrun when my opponent had no creatures, worse still the force was payed with 1 of my 2 morphlings (50% of my win conditions RFG ).

Heh these are hilarious... makes me feel a bit better. I've -
Repeatedly played CoB while at 1 life, while my opponent had Rishadan Port - he had to warn me not to so I wouldn't lose outright (casual envr).
Dropped Zuran Orb to stave off Stompy while my opponent had out inactive Hidden Guerillas.
At Onslaught Prerelease, newly back into the game after ~8 yrs, I attack, and chain of plasma a 1/4 wall - my opponent asks "damage on the stack?" We enter a discussion about what the hell the stack means, which results in him letting the wall take the damage, chaining my spell back to kill the attacker the wall was blocking and let the wall survive unharmed. (A few turns later he plays out Quicksilver Dragon and Words of War. Yeah I lost that one.)
Played Shared Triumph: Beasts against a draft deck with beasts on the table, having a few chump blockers and expecting mine to show up soon. Didn't happen.
Playing Keeper casually and forgetting to bring my SB. You attack? I'll cunning wish for... uh... ah freakin hell...
Many, many others I have thankfully forgotten...

heh, glad i'm not the only one to make stupid mistakes. Many years ago i played against a friends 30counters.dec. He had 0 cards left in his library, i had a full hand of goodies and plenty of mana on the table. Just to show him how much control i had, i played out a Nevinyralls disc. He responded with power sink for 13. Oh, did i forget to tell you that i was at 1 life with 3 CoB:s out ? :/

Demonic Tutored for a card, somehow missed it the first try, then i somehow figured out in my inzanely dumb head that the effect had ended, then played a Vampiric Tutor after the same card, to finally realize this was a different deck then what i thought it would be.
I dont know how it happened, i just failed in so many ways, i still laugh out loud when thinking about it.
Second; i had a piledriver, 2 1/1 goblins and a raging out, turn 4, i choose not to attack (and win) to have blockers for the next turn. I didnt realize what i done before i left the tournament.. 3 hours later

Hmm....
My opponent: Drop Loa, go.
Me: Pitch Wasteland, Mox Diamond...
I still won, but it was frikkin close. That LoA was active for atleast like 5-6 turns. Thank god I got my Tax/Rack engine up pretty quick to counteract it.
I also constantly make mistakes when I play Fish against my friend's Stax. I always sac my Lands without floating mana to activate my Mishra's Factories.

Opponenet- Basking Rootwalla
I'm playing Hulk, and I have Wish in my hand to fetch out a huge Yawgmoth's Will after casting a Vampiric Tutor to win next turn with a Walk/Lotus in my graveyeard. He swings, and I take it, he pumps. I smack my head on the wall, and proceede to lose misiberaly with a Vampiric Tutor in my hand....

Over apprentice: Me playing old-school academy. Him suicide black (with zombies and lake of the dead). He had 4 cards in library with a hand of 7. I impulse. I see Windfall, Timetwister, Mox, crop rotation.................I choose Timetwister. Luckily, I conned him into thinking I snatched the windfall.
In Real Life (T1): Opponent playing some B/W aggro deck. Me U/R phid (unpowered). I have out all 3 of my sided in propagandas. I activate Nev's Disk....................needl ess to say I promptly lose and trade off the disk for another keg.
(T2): Trying to regenerate creatures in affinity after they cast Wrath of God. The worst thing is that I used to own a playset of those and still forget the "cannot be regenerated" part.

I'm playing type two slide. I'm in the second game of the last round of a tournament, I need to win it for the better tiebreaker to top 8. My opponent has a visara in play, I have an angel. I'm at five life. My opponent taps visara at the end of my turn to kill the angel. I TAP OUT to slide out my angel. Next turn I have no angel to block, and no mana to use the slide.

I made the worst play in all of the history of magic once...when I was playing a former member here who was once known as Flamekatana.
I was playing Ankh Sligh and he was playing Tendrils Academy. He played a Rector and even said "watch, he's gonna bolt it"...and I did. I have no reason why. But I bolted a rector, and it lost me the game.
I also, Blocked a Nantuko Shade with a Jackal Pup...forgetting what happens when it took damage. And I ended up taking 8 damage and I lost a Jackal Pup...oh god how I hate myself.

This is my friend's mistake (player B is my friend)
Player A trix (no rectors):
3 lands, 2 mox, sol ring, 4 cards in hand (ritual, donate, brainstorm, mana crypt)
12 life
Player B sligh:
2 mountains, 1 jackal pup, 1 mogg fanatic, 6 cards in hand (3 reb, lightning bolt, incinerate, mountain)
Player B attacks for 3 damages, plays mountain and.... plays bolt and incinerate... 6 damages...
Player A eot plays brainstorm, draws illusions, and win

my opponent was at 200+ life or something, some kind of waky combo with: Sneak Attack, Y. Bargain, WOrds of Worship and Fastbond. He has no cards in his library and 2 huge serra avatar's in play. I can't kill him no matter what I do, I hold on because with my madness there's nothing he could do to me. (funplay, had 2 sporefrogs in play, and two genesis in graveyard, he couldn't kill me  ). The game continues and I only have 2 cards in my library... Then I finely look at my graveyard and hand, both of them contain a deep analysis.. I read the card: target player draws. LOL game. That game took me 35 minutes to much I could have done that in turn 10 or something but I didn't read the card . 2nd, blowing a powder Keg with suicide vs fish, he had 3 1/1 flying and I had two shades in play..... (hello my name is stupid).

well, when I was playing GAT against greencrap.dec I gushed my two lands back to my hand floating UU, and thus had no green to cast the berserk in hand, which i neglected to realize until after i had dropped my hand and removed my graveyard.
and my most painful mistake...back in the day i was playing in a sealed deck (Ice Age starter + mirage/visions boosters) tourney for a black lotus. I was in the semi-finals with a pretty strong UB deck with hella strong board position. I was about to seal the win with some serious Soldevi Simulacrum beats. Unfortunately I forgot to pay the upkeep next turn, got flustered and proceeded to lose. Then, I repeated the mistake in game two. I later learned the lotus was conterfeit, but still...
