Once Sundering Titan declares targets, if I Gush tageting those lands, what happens?

Does Sundering Titan fissle?
Does controller choose new targets?

You move the lands to your hand, and titan stays in play.

Umm...Sundering Titan doesn't target...

Upon resolution of the Titan's comes into play ability, the controller of Titan choses lands to destroy.  At this point, you can't do anything.

You can Gush in response to the abillity, but they still chose lands after that.

The question is totally moot. You don't choose the lands to destroy until the Titan's ability is resolving, and you can't play a Gush in the middle of the resolution of a spell or ability.

'Choose' is not the same as 'target'. If it targeted the lands, your trick would work. But it doesn't.

EDIT: Damn you, Jebus!

So my best option would then be to Gush before Titan resolves?

Before the ability resolves at least.  You can keep two Islands that you really want to keep.

However, they still have free choice over everything else.

Just to clarify my own usage from my decklist, where people may have gotten confused, the idea is to return 2 duals to your hand and leave the Cities, Wastelands, Fetchlands, and the 1 or 2 duals left over for the titan to choose from.
