Living Nightmare

Whenever a card is put into an opponent's graveyard, put a night counter on Living Nightmare.
At end of turn, sacrifice Living Nightmare and put an X/X black Horror creature token with fear into play, where X is the number of night counters on Living Nightmare.

This could be very hard to track during a Timetwister or such. What if it were just */*, where * was the number of cards in target opponent's graveyard?

Exactly. You cast Timetwister (or Repopulate, or whatever graveyard-messing spell) and then cast this guy. It's a pain in the ass to figure out how bis he is, since you can't just look at the graveyard.

Hmm. To avoid that, how about making it proactive instead of retroactive:

Whenever a card is put into an opponent's graveyard, if Living Nightmare came into play this turn, put a +1/+1 counter on Living Nightmare.

Or how about:

Living Nightmare

Could this not be at {3}{B} so that it's more friendly for other decks it could be abused in?  Or are you trying to make sure it stays in Black for flavor?

Corrections - it needs to create a creature "token", not just a creature. And originally it counted cards that went to the graveyard "from anywhere"; just checking whether you intended to change it.

I wanted it to be cards from anywhere.

I am also cancelling the 24 hour clock.  I think that this card is a bit too weak.  I personally think that it could be safely costed at {1}{B}{B} or even {B}{B} because it does nothing and is a waste of a card if you don't have an assload of support for it.
