This seems to be a card combining too much random effects. I don't like it. Narrow it because as it is now, it doesn't make sense.

This card is like if Final Fortune, mated with Psonic Blast... and both of them were cousins.  This is a card for sucide red and thats about it and even then i hope those two cards you just drew were fireblast or you lose.  But I must say there is an air of arrogence when you play this card:

Me: I'll play Desperate Exertion
Them: LOL ok I win
Me: Bolt, Bolt, Blast, Blast 0wned

you could do that fourth turn 4 red mana, tap 1 for this card, tap 2 for two lightning bolts and then sac all four mountains for fireblast now that would be pimp. (4 damage + 3 damage + 3 damage + 5 damage + 5 damage = humiliation)

@Guardian - I don't think that the effects are that random.  This was an attempt at screwing with the final fortune concept.  Instead of the extra turn, you net two cards and 4 damage at the additional cost of 7 life.  Although the wording might be a little awkward (this is my first attempt at card creation), the effect follows in the footsteps of stupid red "gambit" cards.

@xthepunisherx - The card seems pretty blanced to me, even if it does have a crappy effect...i love to beat people with burn decks.. this is what came to mind  .  They can always pack a counterspell.

This card's effects are VERY random. It's obvious that you just took a normal card and tacked on bonuses and drawbacks until it was about balanced.

Make it an instant and make it so you only lose 5 life.  Lightning Bolt wasn't that broken, was it?

This doesn't seem exactly like a red spell. Loss of life is traditionally black. Maybe I'm just anal, but this would feel more red if it read, "Desperate Exertion deals 4 damage to target creature or player and 8 damage to you."

I'm not sure I like this card, though. Bolt variants aren't exactly exciting and the more penalties you tack on, the more inelegant they seem. I will be honest, this doesn't feel like a card for which you asked, "How can I make an interesting red spell?" Rather, it feels like you asked, "How big of a penalty do I need to make a 4 damage for burn spell?" To tie this into another topic currently going here, a burn spell is a card that's best designed around a block flavour mechanic. It's seldom going to be a super-interesting spell, but it will at least mesh with the local flavour.
