I am loving this deck, with the following adjustments (although they are ofcourse metagame dependant):

MD:
-2 Stifle
+2 Deep Anal

I didn't like stifle too much, it's just too random for me. Deep Anal has always treated me nicely, and it's the perfect drain sink.

SB:
Big Changes here, basicly since I don't want to be as wish dependant with only 2 wishes, and I'm afraid of some matchup (like Dragon, and workshop).
My SB looks like this:
SB:  3 Red Elemental Blast
SB:  2 Fire/Ice
SB:  2 Pernicious Deed
SB:  1 Rushing River (subject to change!)
SB:  3 Coffin Purge
SB:  2 Artifact Mutation
SB:  1 Naturalize (subject to change!)
SB:  1 Berserk
Things I'm gonna try: 1 Smother, and 1 Stifle/Chain of Vapor.

I concur with Morefling.  The stifles were too random for me as well and I didn't feel them to make a big enough difference for the game overall to warrant the MD slots.

My changes;

-2 stifle

+1 Cunning Wish
+1 Pernicious Deed

I still think that the 3rd wish is a must have especially with the environment this fast, the berserk needs to be ready at all times while still having another wish for utility.

The MD deed is just a reset in case the game drags or the initial draw doesn't pan out the proper amount of answers and Mr.Teeth can't handle things on his own.  I am quite open to suggestion as to what would be a better 1 slot for removal or utility but have not come up with anything better than the deed yet.  As well, it also frees up another much desires SB slot.

In regards to the sideboard, I didn't think the terminate was that much better than smother to warrant the chances of its inconsistency with the 2 secondary colors.  Additionally, I also agree that Naturalize should still be in the SB just because its so versatile.

I doubt you need a 3rd cunning wish to 'be sure' to grab a Berserk. You can often win without Berserk (at least, I know I can with my extra deep anals). Wish is, although very good an needed, a little too slow against at least half the decks in the format.

I never got the Terminate either, since basicly any creature that matters on opposing side is either a Tog or a Dreadnought. Well, in 80% of the cases.

I know that the wish is a bit slow but whenever i play Tog with just 2 wishes, i can't help but want another most of the time.  

For example, when I tested against Mud and I had the tog ready to kill lots of times, but then my opponent dropped  another threat with which to prolong the game, so I use the wish to stop the threat but the game is still prolonged because now I can't berserk the tog.  Situations like these just keep telling me to keep the 3rd wish.

For reference this is my SB at the moment;

1 Berserk
1 Naturalize
1 Smother
1 Coffin Purge
1 Deed
2 Tormod's Crypt
2 Deep Anal
3 Red Blast
1 Rack and Ruin
1 Blue Blast
1 Artifact Mutation/Gush- very metagame dependent
