Too much to remember I'm afraid. One of the worst must be this one:

Back in the day just when Legends was just released I played Counter/Burn and started a game against 'the biggest rip off new players that ever walked the earth'. He Disintegrated me for like 8 or so, and that was all I had left. 'Don't worry.' I remember thinking to myself. 'I will backdraft like another friend did earlier that day'. And so I did. I told him he would take 4 instead. He looked at me puzzled and said: 'Well, you're dead.' After some discussion he got it through my head that although he would get half I would still get the rest. That last part I had missed completely. What a sucky card!  

Ah well the friend whom used it before got the same lesson from me later that day.  

Besides, the coolest thing I ever did was playing Red/Blue/White (a bit of an pre- The Deck kinda thing) against 'the biggest rip off new players that ever walked the earth' whom was showing off his Gauntlet of Might Gobbo deck. I didn't knew the card even existed and had to read it when he put it on the table, getting oh's and ah's from the audience. After I finished reading I simply disenchanted it. He begged me not to do it so he would have a change to play with it. Offcourse I did it anyway because he was a complete basterd!!! Then someone pointed out how cool it was to kill the $150 (Back then) card with a $0,10 card.

It was a long time ago but I'm remember it because it costed me top 8 in a tourny.  Black Vise had just been restricted but I forgot to take out copy # 2,3, and 4.  I had one Vise out and the opponent was at 4 life.  Played my Wheel of Fortune from my hand and drew another Vise so I dropped it down to ensure that he would take at least 4 damage on his turn for the kill.

He pointed out it was restricted; my response, "aaaah shit!"

Thing was, I was screwing people all day with multiple Black Vises and nobody said anything.

I was once playing a fully powered keeper with my cheap ass black/blue control deck that I had made up of leftover cards, and I had managed to overpower the real keeper, but then he dropped a moat which put the game in a stalemate.  Then he played something that I didn't like, and I scryed for a counter and countered it...which I realized was the cockiest and dumbest thing I've ever done, because we just played draw-go for the next half hour and I lost because he had 1 card left on his library!

I was playing with one of my friend's land kill decks and managed to stone rain 4 fetchlands.  After I stone rained the first one my friend told me that he would just sac it in response.  I was like "Oh shit.  Forgot about that."  Then i proceeded to make the same mistake 3 more times.

My opponent was playing Goblin Sligh. I was playing B/g Void. My opponent swinged for 17. I had a Pernicious Deed in play and I chose not to pop it until after I took the damage. I lost the game next turn.

I had a wasteland in play, my ooponent drops a wasteland.  Nothing happens, next turn I draw a sinkhole and sinkhole his wasteland, he wastes miine in response.
Once on a fact or fiction put yawg will in one pile, other 4 in the other.  He takes will and wins.  Ughhh!  Once I drew 6 instead of seven to start thegame, had no idea until the third turn that I'd done it.  Oh well, thats all for now.

chalicing for 2 while playing chalice black against gay/r.  Yes. stupidity at it's best.  Already 2 and 2 and it's the fifth round.  tired....not thinking straight.... he drops red mana production, lavamancer.  He passes ~ i drop negator, think, scoop.

I remember trading at a tournament many years ago (~1996).  A guy was trying to get some dual lands from me and all I wanted from him was his Library of Alexandria.  He wanted 4 of my dual lands, and I felt I could only give him 3 + throwins...  We didn't trade, lucky for him.

If you changed it during your upkeep... I am not sure on this ruling, actually... both Arcanis's would be in play already... so wouldn't both die? Or neither?

One time I sided in Spinning Darkness instead of Edicts against Suicide black. My thinking at the time was that both kill critters, but the life gain from the Darkness will help against aggro. I only later realized my mistake when I was stuck with 2 swamps in play, a Darkness in hand, and a Hippie on the other side of the table. That friggin Hippie went all the way.

As bad as that is, my worst play ever happened later that same day. I'm facing Sligh, and my opponent double bolts my Masticore when I have only 1 swamp untapped. I Dark Ritual to save it, burn for one, then promptly forget to pay it's upkeep next turn.

Worst play? Seven years ago, playing some weird R/G aggro deck I did the most brutal first turn play:
1- Play city of brass
2- Tap city of brass for G
3- Proceed to play Ghazban Ogre
I didn't have the fourth Taiga, so I used CoB instead. That happens when you use suboptimal cards...
Another misplay I'll never forgot happened two or three years ago, I was playing with some sort of U/B control deck and was at low life. It was my opponent's turn, who was playing suicide. Still I dont know what I thought, but at her eot I intuitioned... ˇˇfor 2 nether spirit and another card!! Obviously she put both on my grave. All my win conditions left...

Playing Hatred on a Dauthi Slayer w/ Spinal Graft on it, WITH the win next turn to make top 8 in a PTQ for Tempest block season a long time ago...

Attacked and pumped my Nantuko Shade when opponent had a Solitary Confinement in play.

Here's another one, which is not really a mistake, but not the most intelligent either. Starting hand: Dark Ritual, Dark Ritual, Dark Ritual, Chalice of the Void, Demonic Tutor, Swamp, Another card.

I play swamp, Chalice for 0. This gets Force of Willed.
I play Dark Ritual, Dark Ritual, Demonic Tutor for Negator, Phyrexian Negator.

Next turn it gets Smothered.

What I should have done follows. This is just theory and I've come up with it afterwords but the fact that I actually got this hand means it's a possibility:

Swamp, Chalice for 0 gets fow'd.
Ritual, Ritual, Ritual, Tutor for Yawgmoth's Will, Will, Ritual, Ritual, Ritual, Tutor for Chalice. Chalice for 3. Chalice for 0 (or possibly 1).

Case in point: I was facing a fullpowered HulkSmash.

I played a March of the Machines in a booster draft tournament, to make my 3 cc artifact  into a creature.  I killed of 3 of my artifact lands.  And I was winning!

I was playing unpowered RectorTrix at a tournament, with Rector in play and Therapy in the 'yard. I announce that I'm Therapy-ing, name the card, and I don't get Bargain because I announce the Therapy before removing the rector.  

Sorta stupid.

Here's a really bad one...

I dark ritual, then Duress my opponent.  It resolves showing a FoW and 2 Misdirections.  I take the FoW.  I then proceed to HYMN TO TOURACH him with the remaining two mana, which of course gets misdirected back to me.

The most powerfull mistakes I ever made was in a tournament (Clichy #2) when playing Sui.

The First one was on T1 when playing first. I played Swamp + Dark Ritual + Duress + (...) I was going to play a powder keg against a GaT/r but I don't know why, I played a Sinkhole on... my own swamp which was the only legal target, *sigh*

The second one was on the same matchup (was really bored and tired) when I wanted to Edict a Dryad which could give me the win and ended with another Sinkhole on his lone fetchs, *re-sigh*

I was playing Sui and my opponent played landstill. He Duressed me, me with a Swamp and Consult in hand. I Consulted in responce, for a... Hymn. I won that though.  

Another Suicide scrubbing: I was playing against a monoblue Stasis and we both had big hands and nothing but basics in play. I drew no creatures and decided to play Chalice fo two to cut off his Stasises and Drains. I soon realised that the plan was not among the best when I started drawing Shades and Wretches...

In a Mirrodin draft I attacked with my Cathodion. My opponent said "you must be holding some kind of bump" while calmly blocking with his Glissa. "What? It 1st strikes! Oh f****.  Better read cards next time." Of course I had no Predator Strike nor similar. Later I forgot to counter his Bonesplitter with an Annul in my hand, which cost me another game.

My friend once played Lotus, Will, Lotus, Twister in his 1st turn. Maybe he didn't like that Will in his deck.

I had Grim Monolith + Power Artifact, at least 1 each of seperate black & blue mana sources in play, Stroke of Genius + Duress in hand, with nothing else.  I don't duress, just go right ahead and play the Stroke.  It gets misdirected to me.

Other "high"lights include forgetting to add the 5th counter to my powder keg during my upkeep while at 3 life and cringe as Superman walks over me the next turn.

I got a new one. I first-picked a Reiver Demon over Bonesplitter in Mir/Mir/Mir draft. Not only that, I then got hosed out of what little good black there was.

The most stupid play in Magic I ever did. I was playing GAT, I played the Tog and my opponent who was tapped out began talking to me...I can't even remember what the conversation was about, but I know it wasn't about Magic. A minute or 2 later he said, "So, are you gonna kill me this turn or what?"...And like a moron I said yep...Gush...Gush...Cunning Wish for Beserk...Discard hand...Remove graveyard...Cast Berserk...Annouce Attack...Opponent Grins and says "I do not beleive the Tog has haste." I felt like a moron, but I did congratulate the guy because that was the best play I have ever seen anyone do to turn a losing game into a winning one. Just goes to show you the game ain't just about the cards.

I remember another of my "I can't have played this game before"  turns, that went like the following(I didnt start):

Tropical Island, fastbond, city of brass & city of traitors, traitors for a monolith, tap monolith, tapped city of brass for red, playing burning wish for a Dark Ritual, which my cleverly thought out deck had in sideboard. Uh, genious at best. Only, to realize that i CANT get my Ritual with a burning wish, and i end up having to floating in my pool. Well, that was not really the problem. The problem was it was a 1-1 game, he playing the suckiest deck and he had seriusly had the most amazing draws i ever seen. It actually won by using tendrils _twice_.. Yes, it did surely suck. The deck couldnt It was about the Top8, i laughed, i could handle the 2 damage.

He had a mountain out, and his hand was: 3 ornithopters, 1 shield sphere, 1 wheel of fortune, 1 swamp, 1 ritual. Oay, that is just wtf. And he actually drew the bloody ritual. Well, he played his 4 0cc critters, the swamp for ritual, tapped the mountain, wheeled into one ritual, one tendrils and the last bloody ornithopter. 9 spells, enough to beat my ass.

This may not be a bad play, but it was more of the amazing luck some people in some situations could possibly have. The deck did suck, it was just a bad tournament. The worst? He had 5 0cc critters in the deck. Take a count  ...
